What should you expect during your consultation?
All of our new patient consults are scheduled for an hour. We want to hear your story and understand what your goals are for your body. We will complete a detailed assessment that will include a comprehensive history and physical along with a photographic assessment of where you are begging this phase of your journey. You will receive a detailed surgical plan and a set of post-operative instructions. Our goal is to provide you with a better understanding of your options and alternatives. Our surgical plans are individualized because every patient’s goals are different for their chests. We want to provide you with information that meets your expectations and helps you make an informed decision. Your post-operative care plan is designed to optimize your outcomes.
We have always provided a comfortable and safe environment, however during this new season of the COVID-19 pandemic your safety and our staff’s safety are paramount. All necessary precautions against Covid-19 virus in accordance with state guidelines are enacted to ensure your health as well as our staff’s health. Every person who comes into our clinic will have a temperature screen and complete a symptom questionnaire. All patients who decide to have surgery are tested to ensure that they are not carrying the virus at the time of their surgery.
What are the surgical options?
There are two types of surgical techniques that are favored for masculinizing top surgery: Either a double incision mastectomy (which removes all of the breast tissue) with repositioning of the nipple or peri-areolar tissue resection with liposuction for chest contouring. The choice of procedure is dependent on the amount of breast tissue, amount of excess skin and nipple areolar complex position. No two patients are alike hence every procedure is catered to the patient’s body type and goals.
What will my incisions look like?
The peri-areola incision hugs the border of the color change of the incision. This approach is suited for patients with a more modest amount of breast tissue.
With a double incision mastectomy – the incision is designed to sit in the crease of the breast and also uses a tailoring technique to remove some of the vertical skin to create a dart. This technique is often combined with a free nipple graft to add color and texture to the breast which helps to normalize the appearance. Some patients prefer to use a tattoo technique to create the appearance of a nipple.
With any surgery, there will be a scar. We take a preventative approach with the type of suture that we use to close the breast and also use a preventative dressing to help protect the scar as it heals. Scar prevention and scar care will be discussed during your consultation.
The ultimate goals of this surgery include the removal of breast tissue and excess skin, repositioning and reshaping of the NAC, and minimization of scaring.

What is the recovery like?
Surgery is performed on an outpatient basis, most often in our own, on-site licensed surgical center. We have a dedicated team that understands the needs of the transgender community and are passionate about your care. Within 4-6 weeks, all incisions typically have healed and most patients have resumed their normal level of activity.
Is TOP surgery painful?
Pain is not the biggest complaint after Top Surgery as most patients report that they have a lot of “tightness.” Sure, everyone feels like they had surgery, but because we also use a long acting nerve block much of the pain can be suppressed. We also provide everyone with a long acting anti-inflammatory (Celebrex) to limit unnecessary inflammation along with a shorter acting narcotic to be used only if needed. Most patients have minimal pain and can return to regular activity in about 2 weeks. If undergoing a double mastectomy incision pattern, drain placement may be required to prevent fluid buildup.
